Our mediation referral form is the most straightworward way for solicitors to refer clients to Family Mediation. Please fill in the mediation referral form below. Once completed, simply press send and a copy of the form will be emailed back to you for your records.

Please note our office will be closed for the Festive Season from 5pm on 23 December 2024 to 9am on 2 January 2025.
